Schedule/Travel: This position is full-time (40 hours/week) Monday
- Friday. Employees are required to have flexibility to work any of
our 8-hour shift schedules during our normal business hours of 8am
- 5pm EST. It may be necessary, given the business need, to work
occasional overtime. This position will require a candidate to
travel and/or reside in and around the Adams, Athens, Belmont,
Brown, Delaware, Fairfield, Franklin, Galia, Guernsey, Highland,
Hocking, Jackson, Knox, Lawrence, Licking, Meigs, Monroe, Morgan,
Muskingum, Noble, Perry, Pickaway, Pike, Ross, Scioto, Vinton, and
Washington counties of Ohio area.
This role is primarily telephonic from a home office, but does go
out into the community for required face-to-face community outreach
events as needed.
If you reside near Dublin, OH, you will enjoy the flexibility to
telecommute* as you take on some tough challenges.
Primary Responsibilities:
Create a positive experience and relationship with the member
Keep the member out of the hospital by supporting regular visits to
their primary physician
Keep member actively engaged with their primary physician
Support the member to ensure pick - up of their Rx
Proactively engage the member to manage their care
Provide member education
Support transitions of care
Help to keep members compliant with their care plans
Partner with care team (community, providers, internal staff)
Knowledge and continued learning of community cultures and
values
Conduct member assessments

Qualifications:
High School Diploma/GED (or higher)
1+ years of experience working with children in custody, adoptive
children and/or the pediatric population
Intermediate level of knowledge of culture and values of community,
and familiarity with the resources available in the community
Must reside or be able to commute within Adams, Athens, Belmont,
Brown, Delaware, Fairfield, Franklin, Galia, Guernsey, Highland,
Hocking, Jackson, Knox, Lawrence, Licking, Meigs, Monroe, Morgan,
Muskingum, Noble, Perry, Pickaway, Pike, Ross, Scioto, Vinton, and
Washington counties of Ohio
Ability to travel locally up to 25% of the time
Access to reliable transportation & valid US driver's license
Preferred Qualifications:
Bachelor's Degree (or higher) in social work and/or healthcare
administration
Community Health Worker (CHW) Accreditation
Experience working in Managed Care, Behavioral Health, Substance
Abuse and Medical Care
Knowledge of Ohio Medicaid & Medicare population
Bilingual fluency in English and Spanish
Field based experience
Soft Skills:
Excellent communication skills
Ability to work autonomously and ability to self-direct
Ability to remain focused and productive each day though tasks may
be repetitive
Ability to work in multiple systems and applications to complete
daily tasks
Telecommuter Requirements:
Required to have a dedicated work area established that is
separated from other living areas and provides information
privacy
Ability to keep all company sensitive documents secure (if
applicable)
Must live in a location that can receive a UnitedHealth Group
approved high-speed internet connection or leverage an existing
high-speed internet service
Physical and Work Environment:
Frequent speaking, using hands to conduct outbound calls, sitting
and standing, getting in and out of vehicle repetitively, use of
hands / fingers across keyboard or mouse, handling other objects,
long periods of close vision looking at a computer monitor

The salary range for this role is $19.86 to $38.85 per hour based
on full-time employment. Pay is based on several factors including
but not limited to local labor markets, education, work experience,
certifications, etc. UnitedHealth Group complies with all minimum
wage laws as applicable. In addition to your salary, UnitedHealth
